Admission Requirements:

Blessed Sacrament Child Development Center is open to all children regardless of race, ethnic background, or religion. Children may be enrolled anytime during the year.

 

All enrollment forms must be complete and filled out prior to your child attending:

• Completed Application and Fees

• Up to Date Immunization Form

• Health Assessment

• Emergency Form

• Medication Form if Applicable

We have provided the necessary forms for enrollment here for your convenience. Copies may also be picked up and filled out at our office during tours or visits.
